To initiate the contracting process, complete the New Behavioral Health Provider Form below and include your CAQH number. E-applications are available for expedited credentialing. You may opt in for an e-application when you complete the New Behavioral Health Provider Form. If you choose not to opt in for an e-application or you do not have a CAQH number, please also fill out either the Oregon Practitioner Credentialing Application (OPCA) or Washington Provider Credentialing Application (WPCA) depending on your location.

If you include your CAQH number in your application, please make sure it is up to date and Providence Health Plan is on the list of insurance companies who have access.

Claims and benefit information
Get access to claims and member benefit information by creating a OneHealthPort account.
Please Note: Effective 12/31/24 Providence Health Plan will be moving ProvLink from OneHealthPort to Availity Essentials Single Sign-On (SSO).
Providence Health Plan will transition to the Availity Essentials SSO effective 12/31/24. You will need to have an Availity Essentials account to continue to access ProvLink after this date. If you do not have an Availity account, you can begin the sign-up process here. Your experience in ProvLink - including the site's functions and features - will go unchanged through 2025, as we prepare a full transition to Availity Essentials.

Providers outside of Oregon/Southwest Washington
Providers practicing outside our territory of Oregon and Southwest Washington may provide services to Providence members, but you must be contracted through Carelon, who manages our national network.

- Educate providers on administrative policies and procedures
- Communicate changes and updates that assist providers in the efficient administration of our plans
- Advocate on behalf of our providers when working to resolve an issue
- Initiate the credentialing process for new providers
- Represent Providence networks in a knowledgeable and respectful way
- Assess the changing needs of a particular service area
- Manage changes to the demographic information of participating providers

Providence Health Plan Provider Portal Update

Are you brand new to Availity Essentials?
The first step to getting started with Availity Essentials is to designate an Availity Essentials Administrator for your organization. Your Administrator is the person responsible for registering your organization in Availity Essentials and managing user accounts. This person should have legal authority to sign agreements on behalf of your organization. Register and Get Started with Availity Essentials.

What if I already use Availity?
If you’re already working in Availity Essentials, simply log in to your existing Essentials account starting on 12/31/24 to access ProvLink.

Will this affect claims processing?
No, the transition to the Availity SSO will not disrupt claims processing. All claims will continue to process as is.

Will I be able to access claims and EOP(s)?
Yes, ProvLink will continue to exist, and providers will still have access to claims and EOP(s).

Will there be any changes to CareAdvance (prior authorization submission tool)?
No, there will be no changes to CareAdvance. You will still be able to access CareAdvance through ProvLink.

How do I access OHP/Medicaid member information?
Continue to use the CIM platform with Ayin.
