Providence Care Management helps you better understand your health so you can take an active role in improving it. Whether you need help with understanding a new diagnosis or assistance navigating options for a diagnosis that has been affecting your life for a long time, we've got you covered.

Our team of health care professionals provide personalized support with health conditions including, but not limited to:

  • Asthma
  • Chronic Obstructive Pulmonary Disease
  • Heart failure
  • Coronary Artery Disease
  • Diabetes
  • Cancer
  • Complex health issues requiring hospitalization, rehabilitation, or extensive outpatient therapies
  • Mental health and substance use



Our Care Managers are here to provide you with:

  • Personalized health education about your medical concerns including new innovations, medication therapy, and symptom management
  • An individualized plan developed with you to help you reach your health goals
  • Ongoing one-on-one support
  • Coordination between your providers
  • Advice on general health and lifestyle choices, including nutrition and exercise, to help reduce risks
  • Health care navigation assistance with prior authorizations, caregiver support, dental care, behavioral health care, end-of-life planning, finding services in your area, and much more

Care management services are available to all members at no cost. Participation is voluntary and declining/opting-out may be done upon contact with your care manager or contacting us by phone.

Need help transitioning your care?

Our Care Management team is ready and available to assist you with things like transitioning from one health plan to another, finding and establishing with a new physical or behavioral health provider, coordination between providers, and much more.
